Getting care in Massena

A small North Country village in New York’s largest county by land area (2,800+ square miles, about 106,000 people), where the county is federally designated as a shortage area for Medicaid-eligible primary care, mental health and dental care and residents commonly live 15 to 30 miles from the nearest hub.

If you live in or around Massena, you already know what the numbers say: St. Lawrence County has about half the primary care physicians per person that New York State does, it is a federal shortage area for Medicaid-eligible primary care and mental health, and many people drive 15 to 30 miles for a routine visit. A video visit with Charles Wade, FNP-BC, lets you handle primary care, mental health and ADHD, weight management, urgent concerns, skin problems, men’s and women’s health, children five and older, geriatric and sleep care from home.

Lab and imaging orders go to Massena Hospital or another site you choose, and prescriptions to your pharmacy. You must be in New York during the visit and have a working internet or cell connection.

Access in St. Lawrence County (Village of Massena; North Country, on the St. Lawrence River)

St. Lawrence County is a HRSA Health Professional Shortage Area for its Medicaid-eligible population in primary care (score 16, 8.09 FTE short), mental health (score 17) and dental health (score 18), with a maternity care shortage score of 9. The county has 174 physicians per 100,000 residents versus 485 statewide, and 64 primary care physicians per 100,000 versus 124 statewide (HRSA AHRF 2022). Many residents live 15 to 30 miles from hubs such as Canton, Potsdam, Gouverneur and Massena, with limited public transportation. About 5.9% are uninsured, 26.6% on Medicaid and 22% on Medicare; 22% of households are in poverty and another 28% are ALICE (working but struggling).

Who this helps most in Massena

Telehealth is most useful here for people who cannot easily make the drive: older adults who no longer drive in winter, residents without a reliable vehicle, shift workers at the plants and the Seaway, parents of school-age children, and anyone who needs steady follow-up for depression, anxiety, ADHD, blood pressure, diabetes or weight. It is not a substitute for emergency care. Chest pain, stroke symptoms, severe breathing trouble, serious injuries or a mental health crisis mean calling 911 or going to the Massena Hospital emergency department, which is open 24 hours, or to Canton-Potsdam or Claxton-Hepburn. Procedures, vaccines and hands-on exams still require an in-person visit, and Community Health Center of the North Country in Canton or Malone offers sliding-fee care.

Local health considerations

  • Distance and transportation: residents commonly live 15 to 30 miles from a hub, with limited public transit, and the county’s Housing and Transportation domain is its most severe social vulnerability. Source: St. Lawrence County 2025 Community Health Assessment.
  • Aging population: 22% of residents are on Medicare, the 65-74 cohort is unusually large, and the 60-74 group will soon raise demand for chronic disease and home-based care. Source: St. Lawrence County 2025 CHA.
  • Workforce shortage: 64 primary care physicians per 100,000 residents, about half the state rate, and only two mental health FTEs short of the HPSA threshold but with a score of 17. Source: St. Lawrence County 2025 CHA citing HRSA.
