Western New York’s largest city on Lake Erie, home to the state’s only freestanding children’s hospital, a large refugee and immigrant population, and some of the heaviest lake-effect snow in the country.
In Buffalo, the challenge is less about hospital capacity and more about getting to a clinician on a January morning when the Skyway is closed and the Southtowns are under three feet of snow. Federal data designates the city as a low-income primary care shortage area and a mental health shortage area, so even in good weather new-patient appointments can be months out. A video visit with Charles Wade, FNP-BC, covers primary care, mental health and ADHD, weight management, urgent concerns, dermatology, men’s and women’s health, pediatrics from age five and geriatric care. Orders go to a Buffalo-area lab, imaging center or pharmacy you pick. You must be in New York at the time of the visit.
HRSA designates a low-income primary care HPSA for Buffalo (LI-Buffalo RSA-1, score 19, updated March 2026) and a Medicaid-eligible City of Buffalo mental health HPSA (score 19). Medicaid enrollment was about 30.4% of Erie County residents in 2015-16. Winter: Buffalo’s seasonal snowfall has ranged from 36.7 inches (2011-12) to 199.4 inches (1976-77) in NWS records, and lake-effect bands can shut down travel for days.
Telehealth fits Buffalo residents who lose workdays to snow, older adults who do not drive in winter, parents of school-age children, and anyone managing depression, anxiety, ADHD, blood pressure or weight who wants steady follow-up without the trip. Refugee and immigrant families who need interpretation may be better served in person at Jericho Road or another community health center. Do not use a video visit for chest pain, stroke symptoms, severe breathing trouble, major injuries or a mental health crisis; call 911 or go to ECMC, Buffalo General, Mercy, Sisters of Charity or, for children, Golisano Children’s Hospital of Buffalo. Vaccines, procedures and hands-on exams still require an in-person visit.
