Good to know

Services, screening recommendations, testing, treatment options, referrals and prescribing may vary with your age, medical history, symptoms, where you are physically located at the time of the visit, state and federal requirements, insurance requirements, and whether your condition can be safely managed by telehealth. Conditions and services listed here are examples. An appointment does not guarantee a particular diagnosis, a prescription, a controlled medication, hormone therapy, a lab order, an imaging study, a sleep study, a specialist referral, an accommodation letter or a particular treatment. Treatment decisions follow a clinical evaluation and the provider’s judgment.

Getting care in The Bronx

New York City’s northernmost borough and, for 16 consecutive years through 2024, the least healthy of New York’s 62 counties in the County Health Rankings, with the state’s highest Medicaid enrollment share and the city’s worst childhood asthma burden.

The Bronx has large hospital systems and a strong network of community health centers, yet federal data still designates nearly the whole borough as a shortage area for Medicaid-eligible primary care and mental health. Appointments fill, crosstown buses are slow, and many families manage asthma, diabetes and high blood pressure at once. A video visit with Charles Wade, FNP-BC, covers primary care, mental health and ADHD, weight management, urgent concerns, skin problems and care for children five and older, with orders sent to a Bronx lab, imaging center or pharmacy near you.

You need to be in New York for the visit. If you want ongoing care close to home, Urban Health Plan, Morris Heights and the other centers listed here serve patients regardless of ability to pay.

Access in Bronx County (borough of the Bronx), New York City

An estimated 71.0% of Bronx residents were enrolled in Medicaid (June 2015 to May 2016), the highest rate in New York State. The Bronx ranked least healthy of 62 counties in the 2024 County Health Rankings, the 16th year in a row, and 35% of Bronx children live in poverty. Transit: subway lines run north-south, so crosstown trips to clinics often require buses.

Who this helps most in The Bronx

Telehealth helps Bronx residents who need regular follow-up for asthma, blood pressure, diabetes, depression, anxiety or ADHD and cannot keep losing half a day to travel and waiting rooms. Parents managing a child’s asthma plan and adults caring for older relatives often find video visits easier to keep. It is not for emergencies. A severe asthma attack, chest pain, stroke symptoms, serious injury or a mental health crisis means 911 or the nearest ED: Jacobi, Lincoln, North Central Bronx, Montefiore Moses or Weiler, BronxCare or St. Barnabas. Procedures, vaccines and hands-on exams need an in-person clinic. If you have no coverage, ask the community health centers listed here about sliding-fee care.
