Madison County seat and home of Eastern Kentucky University, about 25 miles south of Lexington on I-75, with one community hospital and a fast-growing commuter population.
Richmond has grown by more than 5,000 people since 2020, and Madison County still has one hospital, Baptist Health Richmond on the Eastern Bypass. In February 2026 HRSA designated the county’s low-income population a primary care shortage area, on top of an existing mental health shortage designation, and the local clinics carry some of the higher shortage scores in central Kentucky. For trauma or specialty care, residents head 30 miles up I-75 to UK HealthCare in Lexington.
A video visit with Wade’s Care First puts primary care, depression and anxiety treatment, ADHD follow-up and weight management on your phone, so you are not waiting weeks for a local opening, and labs and prescriptions go to Richmond locations.
HRSA designated Madison County’s low-income population a primary care HPSA in Feb. 2026 (LI-Madison County, score 17) and a low-income mental health HPSA (score 18); White House Clinics and Kentucky River Foothills Development Council carry FQHC HPSA scores of 21-22, and three Richmond rural health clinics hold their own designations. Madison Service Area has been a Medically Underserved Area since 1995. Richmond grew from 35,164 at the 2020 census to an estimated 40,663 in 2025. Kentucky River Foothills runs Foothills Express, including Richmond Transit buses, Big E Transit on the EKU campus and Medicaid trips.
Madison County’s chronic disease rates sit below the Kentucky median, with CDC PLACES estimating adult diabetes at 11.3 percent, but mental health is a different picture: 27 percent of adults report depression and nearly one in five report frequent mental distress. Telehealth suits Eastern Kentucky University students who need anxiety, depression or ADHD care between classes, commuters who drive to Lexington every day and cannot reach a Richmond clinic during the workday, and families in Berea and the county’s rural edges who would rather not drive for a medication check. Go in person for chest pain, stroke signs, serious injury or a mental health crisis; call 911 or go to Baptist Health Richmond or Saint Joseph Berea.
Kentucky Medicaid, state-specific plans and HMO plans are not accepted. Participation is limited to the PPO plans and TRICARE listed above, subject to benefits verification.
Medicaid: Kentucky Medicaid is run by the Department for Medicaid Services in the Cabinet for Health and Family Services. It covers low-income children, pregnant women, people with disabilities and older adults, and, under the Affordable Care Act expansion, adults 19-64 with incomes up to 138% of the federal poverty level. The Kentucky Children’s Health Insurance Program (KCHIP) covers uninsured children under 19 who do not qualify for Medicaid, in families up to about 218% of the poverty level. Kentucky SKY, run statewide by Aetna Better Health of Kentucky since 2021, covers foster youth, former foster youth and other children in state care. Most members are enrolled in a managed care plan; the rest are in fee-for-service Medicaid. House Bill 2, enacted in 2026 after the General Assembly overrode the governor’s vetoes on April 14, 2026, makes coverage for many expansion adults 19-64 depend on 80 hours a month of work, school, training or volunteering by January 1, 2027, the same date federal law requires, with exemptions for pregnant and postpartum members, people with disabilities or medical frailty, and parents or caretakers of children under 14.
Five Medicaid managed care organizations serve Kentucky in 2026: Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky, Passport Health Plan by Molina Healthcare, UnitedHealthcare Community Plan and WellCare of Kentucky. Anthem Kentucky Managed Care Plan stopped being a Kentucky Medicaid MCO on January 1, 2025, under a court order, and its members moved to other plans. A 2026 state law (KRS 205.5321) extends the current MCO contracts through December 31, 2028. An August 2026 DMS enrollment count showed WellCare with about 376,000 members, Passport about 258,000, Aetna about 216,000, Humana about 191,000 and UnitedHealthcare about 147,000, with about 161,000 in fee-for-service, out of roughly 1.35 million members.
