Getting care in Louisville

Kentucky’s largest city, a merged city-county metro government on the Ohio River and the state’s main hospital and medical-school hub, with a long-documented life expectancy gap between the West End and the east.

Louisville holds two of Kentucky’s three Level I trauma centers, UofL Hospital downtown and Norton Children’s Hospital, and Norton, Baptist Health and UofL Health run emergency departments across the county. Norton West Louisville Hospital opened on S 28th St in November 2024, the first new hospital west of Ninth Street in generations. Routine access is a different story. HRSA lists low-income primary care shortage designations for the Northwest, South Central and Southwest parts of the county, and life expectancy in parts of the West End trails the East End by about fifteen years. A video visit with Wade’s Care First gives you primary care, mental health, ADHD or weight management from home, and any labs, imaging or prescriptions are sent to a Louisville location you can reach.

Access in Jefferson County

HRSA designates three low-income primary care Health Professional Shortage Areas in Jefferson County, Northwest (score 20), South Central (19) and Southwest (18), all designated in mid-2025, plus the low-income ’Centerstone MHCA’ mental health HPSA (score 18). Central West Louisville has been a Medically Underserved Area since 2009, and several Jefferson Service Area MUAs date to 1982-1994. A Center for Health Equity analysis of 2017-21 data put citywide life expectancy at 74.8 years, ranging from 65.4 in parts of the West End to 80.8 in the east (Courier Journal, Jan. 2024).

CVS closed its only West Louisville store, on Portland Ave, in January 2024 (WHAS11), and Walgreens closed four Louisville stores in 2022 (Louisville Public Media). TARC launched a redesigned bus network on Aug. 2, 2026 that cut overall service about 12 percent while adding 15-minute weekday service on Broadway, Bardstown Rd, Preston Hwy, 4th St, Dixie Hwy and Market St (Louisville Public Media).

Who this helps most in Louisville

Jefferson County’s numbers look better than much of Kentucky’s, but they hide wide gaps. CDC PLACES estimates 13.5 percent of adults have diabetes and 37 percent have high blood pressure, and one in four adults has been told they have depression. Telehealth fits the people who struggle most to keep appointments: shift workers at the airport hub and the plants, parents in Portland, Shawnee or Russell juggling a bus transfer, and anyone who wants anxiety, ADHD or blood pressure follow-up without a day off work.

Go in person instead for chest pain, stroke symptoms, a serious injury or a child struggling to breathe. Call 911 or go to UofL Hospital, Norton Children’s or the nearest emergency department, all open around the clock.

Local health considerations

  • Chronic disease: CDC PLACES (2024 release; 2021–2022 survey data; crude modeled estimates) estimates adult diabetes at 13.5%, obesity 37.0% and high blood pressure 37.0% in Jefferson County; 25.0% of adults report depression.
  • Life expectancy gap: 65.4 years in parts of the West End versus 80.8 in the east (2017-21 data).
  • Pharmacy access: West Louisville lost its only CVS in January 2024.
  • Transit: TARC’s August 2026 redesign cut overall bus service about 12%.
